Regarding Patient Visits
We work very hard to accommodate patients appointments without delay. Please arrive 15 minutes prior to your scheduled appointment to allow adequate time for you to complete any required forms and give us updated insurance or contact information. If you arrive very early for your appointment, we will try to see you sooner if possible. If you are more than 30 minutes late for your appointment, you may be asked to reschedule. Patients who routinely miss appointments may be discharged from our practice.
The Challenge of Completing Forms
We understand that on occasion you may need a form completed. We try to complete many of these requests immediately. However, sometimes it’s impossible. Therefore, you’ll be required to fill in as much information as possible before requesting forms from Dr. Cothran. Please allow a minimum of one week for forms to be completed. Note: There may be a fee for processing some forms or for re-processing lost or incorrect forms.
After-hours Emergencies
In case of emergency, always dial 911 before dialing our office. Urgent calls after hours must be made to the main phone number – 847-923-6155. After-hours calls will be forwarded to our answering service, who will page Dr. Cothran.
Prescription Refills
Requests for prescription refills will be processed directly by Dr. Cothran during normal practice hours only. If you’ve been seen in our office in the past year and/or need a refill to get you through until your next appointment, please call your pharmacy and ask them to fax a refill authorization to our office.
You do not need to call our office. Please allow a minimum of 72 hours to process your request.
Scheduling Annual Exams
Annual health exams (and Pap tests) are usually scheduled well in advance. However, we can schedule most appointments within just a couple of weeks, and we’re pleased to welcome new patients for an appointment within a week.
